I want to make sure before I post that I understand how to use Smite Evil.

You add your Charisma bonus (which is +2 on your sheet) to your attack roll (so if you normally roll 1d20+4, you'd do 1d20+6) and +1 to damage (so for the longsword, which is 1d8+3, make it +4).

Shouting something heroic is optional but commonly done. ;)
